Clip Request Form

Use this form to request your routine or dance performance from an event filmed by GlobalDance.tv. There is a $30 charge for each request which must be paid before completing your request. Once payment is complete, we will add your request to our work queue and provide you with a download link to your 1080p video file.

The name of the event where the performance took place. Please be specific and include details like date/time, dance order, and/or heat number (if applicable). Full names as registered or announced at the event. Please use *asterisks* or _underscores_ to indicate your name so we know which competitor is filling out this form. We need to know the best way to get in contact with you! Your order will not be fulfilled until payment is confirmed. Additionally, if you have any further information on the date/time you competed, or what color clothing you were wearing, please include these details so we can find your clip faster. This will be used to communicate with you and send the download link.
